IIRC, first you have to date Lena, then take Holly out to the book fair, kiss her, but no more. Beg forgiveness. Lena then needs to make out with Holly. Then basically what the other person said.
Keep in mind in order for the throuple to happen, Holly cannot be corrupted by Ivy and Lena must pursue her romantically every opportunity while dating Ian which will lead to Holly kissing Lena eventually.

Not necessarily. You don't have to follow Holly to the book fair. But having a good relationship with Holly helps. Just make sure your Ian gets over Gillian before chapter 12 and you should be good.
