I mean you're sort of conflating too different kinks, because cheating/cuckoldry isn't always about humiliation. The other party can just be oblivious. This is mostly true of cheating in Lena's path, where she is usually just guilty about the act. Humiliation develops, but only if the player pushes that.

But it being a closer friend is more about the shock when discovered. It's terrible to find out your girlfriend is cheating with a stranger, it's worse to find out that she's cheating with your best friend. I was making a joke in response to her being a naturally better fit to cheat with Axel, but it is true that Ian offers more nuance because he's closer to Wade. With Axel, it feels like just throwing the hot people together.

I'm narrow-minded when it comes to discussing our fantasies and expectations and if these two concepts divert too much in these instances. Its also why I don't get dreaming about Lena/Cindy or Cindy/Jeremy, when these characters don't show interest in each other, it wouldn't be consistent in their characters, and the story isn't set up for them. For your fantasy, you would need to rewrite the plotline and have Wade and the bull be best friends and bloodbrothers. Perry could fit the latter as his best friend, but obviously he is not able and/or willing to make a move on Cindy. Meanwhile, Ian is written as a much more distanced friend to Wade on purpose, so that getting together with Cindy is not that much of a betrayal. He comes of as relatively sympathetic, relatable and realistic because thats his core character, it would outlandish and inconsistent with his character if he acts as a total selfish bastard and outright villain.
Like everyone, I have my own fantasys and wishes, but if the premise, set up characters don't fit, I see little to no value bringing them up, I'd rather discuss what is plausible, logical and possible. Hence I took your joke on face value and elaborated on why I don't consider it possible, even though in the end everyone here can dream about whatever they want, after all, porn games are nothing but about these wish fullfillments.

It seems that for "pure" Lena/Ian this option is only open if Lena did not tell Ian in Ch.11 that she'd be interested in giving a threesome a try. Which seems counter-intuitive/bugged and perhaps that condition should be ian_lena_3some == True instead, but for the time being it is what it is.

Again, there are degrees to it. That's why even I said jokingly that she should fuck Perry, because we both recognize that Perry is a closer and supportive friend to Wade. But Ian is also a friend of Wade's. In almost every friend group instance, it's Perry, Wade and Ian. Ian and Wade may be closer to Perry in terms of how they act, but Wade and Ian are friends as well. So there would be an extra layer of betrayal to the act of cheating. It would be worse if she cheated with Perry, but obviously Perry would never be able to seduce her, and Cindy doesn't find him even slightly attractive.

So the cheating paths we have for Cindy are Axel and Ian, and of those two paths, only one of them is a friend who should even remotely care about Wade's feelings. So it's more of a betrayal for the one who should care even a little, than one who no one would expect to care about Wade at all. This isn't me projecting something onto the story that I want to be there, I'm talking about what's actually already in the game, and why Ian is the better choice for a cheating storyline because it's shown to have more complexity and conflict. Perry is a joke to bring the concept to it's logical conclusion, but even if you want to talk about what's plausible, logical and possible, Ian cheating with Cindy is already a more bitter betrayal because it involves the betrayal of both Cindy and Ian, rather than just Cindy's betrayal when she cheats with Axel.

So, scratch that, i should've checked the rest of the script. Those conditions in the mod trigger the date ending early.

for "pure" Lena/Ian who meet "their" part of the conditions (Lena has agreed to having 3some in a future) the game will still abort Emma date early, because that Lena/Ian arrangement will trigger the next failure test in that line (ian_lena_couple and ian_lena_open != "lena" and ian_lena_open != "ian") since they are also technically ian_lena_couple but haven't opened their relationship since they didn't experience any crisis.
That last or block should include check for and ian_lena_pure == False, so the pure pair doesn't get included and subsequently kicked out here, if they've passed their own part of the check.

edit: and likely a minor bug in earlier chapter that slipped through:

And if Holly Trinity is activated in Ch.12 then Lena who's been friend with Holly still has friend icon for Holly in her agenda, instead of heart one.

If Lena's with Seymour you will have to delete it if you sign the contract, if she dates Ian that can lead to conflict with different outcomes but besides that I've seen nothing bad coming from setting up the account.
