Lena had to go further with Axel or Seymour in chapter 9. Or you have Ian and Lena date, but never have them fuck.

I never been able to get the cuck route when Lena didn't pursue Axel.
The cuck route can be started when Lena offers Ian a handjob instead of regular sex in Ch.10. For this to happen Lena has to be upset, either due to having sex with Axel in Ch.9 or if she's signed Seymour's contract while feeling reluctant or against it.
Alternatively, this can happen if Lena has fucked Mike or Robert but didn't yet have sex with Ian at this point -- Lena starts to get options to avoid sex with Ian on purpose, if it doesn't happen after few of their dates.
Oh! I probably always have Ian have sex with Lena during that time where you are required to have high lust Ian because it gives him extra skill that needed for him to have it possible for him to keep his job

My Lena only fooled around with other girls (while being in a sexless relation with Ian) and still got the cuck route. Which was a bit dissapointing, because the dialogue was saying that she was fucking hordes of guys, which just wasn't true.
You could always write this off as Lena telling Ian made up shit to press his cuck buttons. It's not like she's contractually obliged to actually have done things she's telling him to get him all hot and bothered.
That said, Lena's past before the game is determined by her attributes. Unless your Lena is all Wits-oriented she did in fact have at least a few dicks in her past.
