Actually there are so many ways I'm not sure how you did not stumble upon it at some point.
Make Ian as friendly to Holly as possible on every occasion.
Make Lena go for all the heart options with Holly too.
Dont make Lena a slut.
Dont make Holly a slut with Ivy.
When Holly tells Ian she kissed Lena, make Ian be ok with it.
When Lena tells Ian about her feelings for Holly, choose the "I care for her too" for Ian.
That's about it.
Of course, if you have made Lena sleep with everyone and made Holly go the gloryhole, don't be surprised you don"t get the Holy Trinity.
The option "I care for her too" is always greyed out when I try the route.
What count as "Lena slut" ? Because I don't think I made her sleep with that many people (Ian, Ivy and red head roommate where the only ones). I did everything else on your list that should be done. I didn't konw there was a GH in the game, let alone you could have Holly go there.
This option requires Ian to show some sort of romantic interest in Holly in the past (whether telling her she's cute, or not shooting down idea of hanging out, etc) but it is not necessary for the Trinity. You can also pick the next option, "I don't mind".
If both these options are greyed out for you then it's because your Ian is still hung up on Gillian (didn't delete her pics, didn't choose to move on) This is something that blocks the Trinity altogether, so it may be one of reasons you can't get it in your playthrough.

Because there is, in fact, a story being told and a reason for why people are where they are.
