That part of the call only happens if Lena cheats on Ian with Ivy in chapter 10, i know that lesbian sex doesn't count as cheating in this game, but i wouldn't call it "pure path", it also doesn't happen on Holly Trinity path.

Technically it can also happen if Lena and Ivy had sex in Chapter 11 instead of 10:

Yeah, I feel that. I usually need a few personal 'canon' runs within my own preferences, but after that I can explore other paths that aren't my thing, but can still be hot. Like cheating/NTR/NTS aren't my bag at all, but if I have enough distance from the MC/PoV character I can still appreciate it, or think of it as an unlikely path for the multiverse version of those characters.

Checking the code, Ivy won't approach Lena if she got completely stopped in Ch.10, but the game also discerns between whether Lena just accepted offer from Ivy to eat her out, or if she returned the favor herself afterwards. The v10_ivy_sex stores that as value between 1-3, and the shower will trigger from 2+

Hmm it seems that if Lena is in relationship with Ian (and Ivy has hit on her in Ch.10) then she'll auto-reject the shower threesome:
