In both walkthroughs it seems to only be possible to be a couple if Lena loves Ian. And the only time you can set that flag to true is when cuddling with her after sex. But there are many dialogues for the situation of being in a relationship without having had sex in the game, so I'm assuming it must be possible? Or do you have to cheat via console for that?

Yes. Chose the love options, but don't if they lead to fucking. Don't invite Ian and don't wake him up with a blowjob.
It will lead to the cuck route and Lena suggests to have a threesome with Wade (even if she is exclusive to Ian).
To get the option to not wake Ian up with a blowjob after bringing him home, Lena has to refuse when she was asked to suck Axel in her dream. If you didn't refuse, there won't be an option.
