Yes. If you get Ian to open the relationship, you can continue to have sex with Axel. Or alternatively, if Lena is able to hide her cheating, you can have an affair with Axel.

First time playing this, so I decided to pursue Allision in my first playthrough. Any hints on how to keep the Alison exclusively to Ian?
You have to tell Jeremy to NOT pursue Alison. Also you have to go for her every single chance you get and never choose to see someone else instead
This might be the most extreme example of “he’s not picking up my signals” I think I’ve ever seen.

Since there is no harem tag, is it possible to have more than one girl in a relationship?
You can regularly sleep with multiple women in a single playthrough. But the only committed relationship with more than one woman is Ian x Lena x Holly.

Two methods with slight difference in who Ian dates first:
Ian dates Lena, is close to Holly and goes on a trip with her, kisses but doesn't fuck her. Lena takes back Ian while constantly choosing LenaxHolly options. This should lead to Holly kissing Lena who is dating Ian. From there the dialogue options for Ian determine if he accepts them both or not at the beach, leading to a threesome and throuple relationship.
The other option is Ian dating Holly, he can also sleep with Lena but not fall in love, and Lena trying to seduce Holly, leading to Holly kissing Lena while dating Ian. Different dialogue will play out, but the idea is the same. From there Ian determines if he accepts them both or not at the beach, leading to a threesome and throuple relationship

The way I did it is have Ian and Lena in a relationship. Ian flirts with Holly but never kisses or fucks her. I didnt even go to the book trip. Have Lena flirt with and kiss or try to kiss Holly every chance she gets. And most importantly have Lena "Protect" Holly from Ivy. Every chance Ivy tries to show her a guy or sends her off with a guy at the club, have Lena stop it. Also, dont drink a bunch at Ivys and instead take Holly back to do the movies at Lenas house. Then at the beach house it starts.
