Ehh, i don't know why; it's just a bit of ribbing done in a group of friends who have just shared a bunch of potentially embarrassing "secrets" of similarly sex-related nature. It's not malicious, because it's accompanied with Lena making a "flirt" face. Similarly, i don't think Ian's reaction on the "non-cuck" path should be any different, or that's it's any weird if he isn't on this path: he's simply embarrassed that his friends have heard about it. We simply don't know how and if Ian can date all three girls at once in the future.
As of now, he ends the romantic path for Alison if he gets together with Lena. He can start a relationship with naughtier Alison in chapter 11, but her chapter 12 event is closed off if Ian pursues Cindy.

If you want to date all three, you have to be in a casual relationship with each of them. Or you can be in a committed Relationship with Lena and cheat with Cindy and Alison. Those are the only combinations available at the moment.
to always avoid? i mean sometimes i need to do the heart option to increase the relation or have certains (like in the club or the photoshoot of cindy)

I don't want to spoonfeed you how to get your specific outcome, especially since its not even feasible. In this game, the women object if their faithful boyfriend Ian fucks another girl. And Ian himself isn't loving every girl equally, so he will and can automatically favor one over the other. You know, like RL.

This game handles relationships a bit differently than what you're likely thinking of -- the status of relationship isn't a numerical value that just goes up/down, but a combination of flags you set with individual choices.
The choices marked with heart indicate you want the relationship with character in question to be of romantic nature. The lust icon indicates you're thinking with your other head and they invoke sex-related activities without any deeper feelings behind it.
