There is an error in the code that ffive could probably explain better that might be the cause.
But how much did you protect Holly from Ivy? Because this sounds like something that's still possible if Holly is heavily influenced by Ivy, regardless of Lena's relationship to either girl.
The sad part is Holly doesn't even need to be influenced by Ivy all that much. Just having Holly sleep with Mark is enough to have her leap in the deep end.
Hopefully it'll be tweaked now that EK is aware of it, if they weren't before.
Lena and Holly did spend time nights with Ivy, lmcluding some girl-girl secenes (shower, Ivy's house), but generally Lena protected or engaged with Hooolly directly. I guess her corruption has more to do with Ivy's off-camera influence than choices made? I do recall Holly went out with Ivy once but Lena wasn't aware/invited. I guess that was it? Makes sense 'be like Lena = out of shell, but hanging with Ivy means corruption? In any case, it's a real continuity issue where in one scene Holly is the most content and loved she has ever felt, and in the next she is chugging on cocks.

I recall this choice at the club. My choice that night as Lena was not to encourage her to go for Mark, but to let her make her own choice. Yes. If you had Holly sleep with a guy (either because she decided it herself or you've pushed her as Lena) that sets off change to "slut Holly". Logically this should've blocked the throuple from happening though (or involve more than just a hookup with a guy, the way it does currently) and there's a possibility it'll be addressed in future game update.
All the better, this way we can determine who winds up as the Daddy either through previous choices, or after having the boys fight that out.
