Ian needs 7 charisma in chapter 10, then he can compliment Louise. She later texts him and he can set up a date. It may not work if he is with other girls, in particular Holly or if he is faithful towards Lena.
This is a long term development that only happens later on and it can happen multiple ways. For example, Ian can date Lena, then fuck Holly and break up with Lena, while Lena also shows interest in Holly.
Yes, currently these two aren't interfering each other if Ian makes a move on both of them.

I'll try and simplify things as possible. If you have any issues feel free to ping me.
1.Ian must have been in at least a casual relationship with Lena at some point. Also Lena_Ian_love should be at 2. You can set it to 2 by cuddling with Ian after having sex with him in chapter 7.
2.Ian must be on good terms with Holly. You can accomplish this by going on those first 2 dates with Holly(The one in the cafe at the beginning of the game and the one in the park) You can also ask Holly to come to her book signing trip with her even if you don't end up going. This will set Ian_go_Holly at true.
3.You can form the throuple from two different relationships i.e. IanxLena or IanxHolly. The most straightforward way is IanxLena. Start a relationship with her in chapter 9. Alternatively for the Holly Path you have to go on the trip with her kiss her and start the relationship as normal on a Holly playthrough (It's worth noting if you want to date Holly, you have to tell Jeremy at the bar in chapter 7 that with Lena you have no strings attached). Ian will break up with Lena to be with Holly but you should have no problems still creating the Throuple.
4.For the throuple to form Lena_go_Holly must be at least 4. Here are ways you can raise it as Lena: By telling Holly she is pretty when she wants to quit pole dancing(+1), by kissing Holly instead of letting Ivy do it t her place(+1), by trying to kiss Holly in Lena's room in chapter 8(+1), by trying to kiss Holly during her photo shoot in chapter 10(+1), by telling Holly you've been thinking about her in chapter 11(+1). By picking the heart option at the cafe when speaking to Holly(+1). In chapter 11, when Holly comes to visit Lena, if Lena_go_Holly is at least 4, i will get upgraded to 5 and Holly will kiss Lena. This will set the flag for the throuple to form.
5.Another thing to note is after recent patches the throuple cannot be formed with slutty anymore
6.At the beach house when Holly confesses to Ian about kissing Lena, select the "Just that?" option and express your support for Holly. From here on out it should be smooth sailing until the threesome forms.

If they're 'experimenting' and Lena has already set up Ian's fantasy for a FFM threesome, isn't he honour bound to try and do the same for her, if she has a MMF threesome fantasy? Let's face it, most of the other options (Robert...he hates him, Mike...he's cheating on his girlfriend, and Wade...still hung up on Cindy) are unpalatable. So Jeremy is by far the easiest and the logical choice. At this point, he's split up with Louise, so no likelihood of complications like the other three.
During the talk with Lena about MMF threesome Ian has text variants which show that he's especially against involving Jeremy in threesome with Lena. To some extent it depends on their earlier interactions (and also how Lena and Jeremy interacted) but the base reluctance is always there. It may be caused by how Jeremy pretty much whole time insist(ed) that Lena isn't a good girlfriend material for Ian.

The most logical choice to me for threesome is Wade, I chose him because I like threesomes with friends which is more meaningful. For one time threesome I would choose someone like Mark or Mike and in general I don't like threesomes with random dudes.

I didn't particularly get that, but then Ian and Jeremy usually have a high relationship on my playthroughs. The way i view it, is that Jeremy is looking out for Ian, because of what happened in his last relationship. And he perceives, with some degree of accuracy(when you consider what can happen), that Lena has a high potential to do exactly the same thing to him. Regardless of whether she does, he's right in his assessment that she's a party girl, and she's always going to get into situations where she's going to be tempted to cheat. Now we can make sure that that never happens, and Lena always resists, but Jeremy isn't aware of that, and he's concerned that Ian may be rushing into another potentially disasterous relationship, that this time may destroy his confidence once and for all.
And I think, Ian surely sees that, whether he agrees with it or not. So yeah, he might be a bit irritated, unless he's completely blind to all of Lena's flaws (in which case he might react much more angrily). But for the most part, I'm not seeing the issue that you are. For me, the most likely thing that Ian is concerned about regarding Jeremy, is the size of his cock, bearing in mind Lena's possible confession that her biggest sexual fantasy involves big black cocks. Mind you, if that's the case, then that's an even more powerful reason for Ian to ask Jeremy. To prove to himself that he's ready to put aside his own fears, to please Lena, and that he trusts that she won't betray him like Gillian did.

It's not me who is seeing the issue. Ian has, for whatever reason, reluctant if not outright hostile (depending on the play through) attitude towards idea of including Jeremy in MMF threesome with Lena. The game in general is treating Ian and Jeremy as mutually exclusive for the most part (if Lena pursues Jeremy it blocks her ability to commit to Ian, and the other way around, Lena in relationship with Ian is largely blocked from getting involved with Jeremy)
Why EK has decided to do it like that is anyone's guess, but it is how it is. Mind you, i'm not saying this threesome is entirely impossible, just that the game's author for whatever reasons doesn't see this trio as a natural fit for a threesome, the way you do.

It is weird how Ian and Jeremy usually have a very wholesome bro relationship when it comes to Alison, both very eager to share her, while Ian is extremely possessive of Lena and her going for both is a breach of trust that even she is aware of, hence she doesn't commit to Ian if she had fun with Jeremy since she know it will hurt him if she goes for his friend.
Its another sign how the story prioritizes the Ian/Lena love as the true one, while all other relationships are mostly variances and secondary, Ian could never love Alison or Louise as much as Lena, just how all of Lenas male options barely have any good chemistry beyond physical attraction (and then there is Stan). Holly is the exception and also a love partner pushed by the narrative.
Ian/Lena - the game simply imposes this couple until chapter 8, no matter how much you resist, a kiss will still happen at the very least.
