Here, when I choose to kiss Holly, Lena always says I almost kissed her, but doesn't.
What do I have to do to make her do it? This scene is from Chapter 8.
Lena is on the path with Ian, and Holly didn't join the pole dancing because I didn't want her to go to the club with the girls. (If I changed it, Lena still didn't kiss Holly.) ???
She won't kiss Holly on that path in that scene. But continue on that path, still choose the kiss option even if they don't kiss, and they will eventually kiss later, leading to the Ian, Lena, and Holly throuple route.
Lena will always hold herself back if she's in relationship with Ian, because she doesn't want to make a mess out of things (she explains it in her internal monologue) But it advances her "attraction to Holly" meter under the hood. If you keep at it, this can result in forming throuple relationship where Holly joins Lena/Ian.
Btw, by all means let her come to the club, this can help her get closer with Lena too and open up a bit. Just don't let Ivy get her hands on Holly, unless you want her to turn Holly into another Ivy.

In chapter 10, when Lena is taking photos of Holly, I still have the choice to kiss her, and Lena says something like, "I really want to kiss her, but I can't. She's flirting with Ian, and I can't do that to her." Does this still push me into the right scene?
Yup, that's still the trinity path. If you can keep Lena and Ian from fucking around and have them move on from their respective hangups on their exes, you'll be able to resolve things during the visit to Perry's beach house, in Ch.12.

And damn, I have a problem, because in chapter 12, while 3 is talking on the beach, Holly says she's bothered you enough. And Ian replies that he hasn't, but he'd like to talk to Lena alone. And Holly leaves, and 2 talks about Cindy, so maybe Ian can't cheat on her with Cindy so he can have a threesome with Holly?
I'm not sure if having Cindy involved messes things up here, haven't tried it myself. It's likely, but the actual decision whether you want the trinity happens later, after Lena arrives and Ian has talk with her about it.
Mind you, this is not just threesome you're getting here, but actual 3-way relationship, that also mostly blocks having sex with other people down the road. At least for now.

How to start a relationship with Holly? I got to the point I had sex with her in the fair trip and then the game forces Ian to stop doing anything with her (no choice between her or Lena).

The stuff about the three of them was just answered, but as for Lena and Holly the main thing is just to make choices where Lena desires her. For example if you masturbate without the toy I believe Lena can then think of Holly, this can be done twice. The relationship progresses similarly regardless of Holly going to the gym or being friends with Ivy, but you should avoid Ivy sexually as well as not be too encouraging about Ivy's attempts at getting Holly laid, if you choose to encourage Holly to go to Ivy's classes.
As for Ian, outside of his romantic involvement he does have some opportunities to influence Holly and her desire to change, so you can choose to do that or not depending if you want Holly to be more or less confident and/or change her style and minor interactions.
If, when talking to Jeremy at Shine, you say you are romantically interested in Lena the games locks you in that route and closes Holly's for Ian, i think.
The Trinity route doesn't allow for cheating after committing to a relationship as far as I'm aware, haven't tried it. Holly's route with Ian certainly doesn't.
Basically,
The main determinant to get the Trinity route is Lena's attraction to Holly plus Ian has to be in a relationship with one of them. I don't think the Lena+Holly route allows for the throuple, could be wrong. The easiest way is be on the Ian+Lena route and, on the first time you can choose who Lena masturbates to, choose Holly. From then on always pick the choice to heighten her desire for Holly, such as kissing her on the girl's hangout at home (either Lena's or Ivy's, depending if Holly is in the gym) and trying to kiss her after committing to the relationship with Ian. If you are doing everything right you should notice that Holly gets progressively more nervous around Lena until ch 11.
Holly going to the gym or not should only affect how outgoing she is as long as Ivy doesn't sink her claws into her and Lena always intervenes when Ivy pushes Holly to get it on with Mark.
Other than that, if going through the Ian+Lena path, I find that going to the book fair with Holly makes the most compelling storyline, as it shows that both him and Lena are interested in her relatively early on (be sure to stop at kissing though).
What I'm really interested in is what triggers Lena smoking pot at the beach or not , is it taking Ecstasy at the club or taking on smoking again?

Which can appeal to some people. It shifts the group dynamic a bit, making Lena the center of it (as she's the primary attraction of both of her partners) rather than Holly.
