It's not open relationship, it's "experimenting together" with emphasis on "together". You can establish an open relationship later in the game, but this requires Lena actually cheating on Ian in a way that comes back to him, so he can confront her about it.

Yes, and you can leave that decision be -- it doesn't have any real impact other than some small mention here and there in scenario where Lena and Ian get involved in a threesome (so far the only option for it is Louise), unless you count Lena/Ian turning into trinity with Holly, but that's a permanent change to the relationship.
I'm curious though, if I go for the threesome with Alison and I'm trying to go for Lena and Holly at the same time, does that interfere at all?
I admit i've never really had Ian get involved with Alison until late in the game, so i don't exactly know -- i think it may be possible for Ian to go for early Alison content, then date Lena (while remaiing friendly/flirty with Holly but not actually doing anything with her) and get the trinity through Lena getting close with Holly. But that's only an educated guess, i haven't checked it in the script.
