I have this strange idea. What if there are trans and femboys in the game as well being able to select what types of genders appear in the game, even making your character a trans or femboy too?

Family members of the MC are not in the game. HOWEVER you can create custom characters that are the MC's mother, sibling, whatever you want. It's under the "Mod tools" tab in the main menu. If you've never done it before, there are guides available.

Do not overestimate those "Relationships". They get overwritten, once you make a GF out of any of them due to the is_girlfriend mechanic for modding traits.

The effort you implyingly request was a topic years ago. You need to fund and articulate your desires direcetly on the main project site (see #1 post) and win(!) some polls on P.atreon (and D.iscord). The market opportunity it represents has no (or insignificant) foreseeable return of investment looking at the current supporting group and may even alienate some already supporting individuals.
Current focus afaik is char dev and corresponding aspects (triggers).

Check your different assistant options for that. One of them is capable of reducing the chances for STD quite effectively. Also the check-up frequency is important to keep an eye on.
i want to DISABLE it, not reduce it, there must be cheat for it

You cant unless you choose what you have been told before. Condoms. It is an essential mechanic of the game.

Follow the instructions. If it is only a character you want, ignore the part about Spawn location (recommended for your first char) and dont change anything there!

@ Mishun - At your office on the first page of the company screen you can set how often your staff will be screened for STD's. Use this, i have average around 50+ Models and at max 2-4 in treatment at a time. Also a good hint - dont boink whores without triple-layered steel condoms and if YOU are the main star of your studio (what i assume) and have a STD yourself you are not a porn actor but a (seemingly rather succesful) STD trader.
