I would try to start it from a terminal window like cmd or powershell. Then maybe you can see an error message.

game.exe is blocked by av or other security software like defender.

I tried and finished the game on version 1.0 when it came out. It was a huge disappointment.

Does version 1.5 add anything worthwhile, like more cutscenes, or is it just a bug fix?

The idea is if you're the one doing the cucking, stealing another guy's girlfriend because of your all powerful chad charisma and sexual prowess then it's okay. Adds to the power fantasy. If you/the protagonist are the one being cucked, then it's sh*t. Most people don't want to put themselves through that. Simple as.

Uh... no? I don't want to disappoint you but this is the truth, it is pretty short, and has way too many chibi art

It's simple, the material has run out. How much can you squeeze out of the same motifs. So many years of the same thing.
Actually the dialogue in Queen Trainer is still quite fun, Akabur is a genius if he can tell the same jokes and they are still funny.
But that's not enough to call this game good.
