If I am reading the description correctly. The theme of the game is:

You have a girlfriend. She cucks you and sends you a video of it... "Normal NTR Game Shit"...
Then goes off the rails?
Then you turn the guy she cucked you with into a t-girl with forced feminization and fuck them as revenge?

Looks like it, but it looks like the NTR is through rape instead of "she thought of you as a lesser man from the beginning" (yeah, this detail is may be important).
Also, you rape the "evil guys turned evil girls" before the rape of your GF happened, since you went back in time.
Oh! And also, everything that's unrealistic happens thanks to an evil god or something like that.

Obviously, that's just our interpretation, since, with this level of MTL, everything is possible, including a full vanilla love story with no rape of NTR at all, or the most evil and unfair NTR of all times. We will never know 'cause the truth is that we can't understand anything at all.
Also, I don't know what the "realistic" part is. Maybe after you finish the game the GF discovers what you did and she interprets you raping other girls as cheating and a crime, and sends you to prison? Without the "transformation" and "time travel" part, since she won't believe such things exist? Or maybe everything was a dream, that you had while traumatized because of the events? And your GF actually tell the cops about what happened and send the rapists to be raped in prison, and she comes back to you and you two go back to being a regular couple that's trying to forget such trauma, just like what happens with real couples when such tragedies happen?
I don't know because I haven't played yet, but "something" tells me I won't be able to know even if I play this game, with this MTL.

To me it sounds like the gf cucked the MC with multiple men.
