The first game clearly has her as the MC's sister, so changing her relation to him in this game changes the whole dynamic of their relationship and the story in a way that simply will not have the same impact as the actual way that it is supposed to be.

I felt that most choices (if they matter at all) were pretty easy if you wanted to go with the "right" ones. Anything where the MC was being humble, not overly aggressive/dismissive, and ones where he was keen to help/be with Mia seemed like the right ones to choose.
