If the game used a image format that supported transparencies, sunglasses would be really easy to add. For some of teh other stuff, given the current state of the game I don't see how it's possible.

You downloaded a modded version of the game. Modders got tired of Oni's quarterly updates that added 2minutes of content so decided to add content themselves such as outfits, locations and sex scenes.
