IIRC [plugged] status does not change without the player doing something to change it. If they're not wearing a plug, they won't start wearing on unless you select the option to plug them while fooling around. If they are wearing a plug, they'll keep wearing it until you select the option to unplug them or select some variety of play that is physically incompatible with them being plugged.

It'd be nice if it were a dirty talk or wardrobe option but it wasn't last I payed any mind to it.

Under flirting, tell her to wear a vibrator. When she's used to it, you can tell her to always wear one under Dirty Talk.
In the 1.6x version, this no longer takes as long as it did in the 1.4x version.

I am not aware of a full Replacement Mod, however there have been a few smaller touch-up Mods posted recently, like some better Teeth and a touched-up Belly. It's not much, but every little bit helps.
