It's in the 3rd scene, after the blowjob section. Are you using toys? IIRC the game won't let you insert if the hole is already occupied. Or maybe dev mode screwed up the scripting somehow.

This game requires locale remulator and WILL NOT work unless you have it or your PC locale is changed. I'm not sure that's the cause though.

The game need to Japanese local, or "Locale emulator" for correct work, do just that. Also you can just find the answer yourself with the search
