It's a rework update, so all that's in this is changes to graphics on the "helping the doctor's other patients" time with the sex therapist.
I really didn't see any difference/improvement worth spending the development time on.

they are upgrading from a series of stillshots at like 2 to 5 FPS to actual real proper animation at 60 FPS with literally identical art. The improvement is vast.

Here is the same scene before the rework.
Notice how it looks exactly the same as your after screencap.
This is because the rework was to change their old clumsy pseudo animation (2 to 5 FPS series of stillshots) into an actual proper animation running at 60 FPS. That is it.
The new animation is vastly smoother which makes it look better simply due to the increase in FPS.
It is objectively better to have more FPS on the exact same art, claiming it to be a downgrade or side-grade is ridiculous.
The art itself looks exactly the same, but with more FPS.

Reworking pseudo animation of series of stillshots into a real animation while looking the same is:
1. very significant improvement. even if the art looks the same, the overall product isn't the same.
2. nontrivial effort
combining point 2 with the number of animations they are upgrading per month indicates they are seriously working at it, rather than just bilking the patrons
also usually a patron bilker would do a reboot for such a thing. They are doing it via a series of patches which redoes a dozen or so animations per month. So you are never losing contents on the update.
that all being said... I would have preferred for them to focus on new content instead of remastering old content.
