I started with AVN and switched when I had to, I didn't notice much difference in gameplay and can't even tell you when it happened. It seemed natural and not that difficult to manage, maybe because I already understood the game by then. I don't believe I've missed anything by not starting with sandbox. I think that ending image was from v0.3.0 (that version had the married endings with Anna, Dora and Moko). So they didn't tie the knot, had no idea. But I believe she was mentioned in a conversation with her brother when he arrived to Mohoro sometime later.
