Either your character gets pregnant, or you help out a woman you find who is pregnant, you need to get a special collectible to get into their mind and purge the broodling inside. The broodling spawns the slugs.
Should be in the starting village. Go into the house with a man standing outside (Left of the Pub). Inside you'll find a woman and a recollection of the Slug boss fight.
Yes I believe it is what you get when you are captured by the demihumans and obey three times.

Yea i triggered this appearance with a debug keybind in the game. I dont actually have any new stauses it just changes my appearance to that anywhere
The slug at the very end explaining to Lilia why she should still have baby making sex with it even after the Otherworlder lost and has no way to fight back now.

According to the gallery, the Seere (hidden boss in the Highway to the demihuman cave) is an Otherworlder hailed by its kind, but it has no interest in breeding Leila/Lilia despite the fact that it can only be found with the special cocaine. Was it successful in entering the Human World, but it's just chilling and waiting for someone capable of spotting it?

Yeah, probably was just chilling his base and then we came and threw around flour everywhere, causing him to get aggressive.
I would also get aggressive if someone came to my home and threw around some dusty stuff

Well, sort of. Technically every birthing scene is tied to where you lose, not where you got pregnant. There is no time passage system in game that would cause a pregnancy to advance and then result in birthing on it's own, you have to take a game over in one of the dungeons that have a birthing scene to give birth. It's a limitation made by how the scenes are handled. If you give birth on the pig farm, it's a pig, if in the goblin cave, it's a goblin, etc.

You need to get pregnant from anyone and then ask Haanja to cleanse the pregnancy which triggers a refight with the slug boss.
