Haanja brainwashed by the cultist?
I've had this bug for a while, And I have just discovered that it's carried over whenever you do New Game+
In the Final Cathedral, make sure that when you start a segment, you finish that specific segment in the same order, or else the names on the dungeon revisit will be wrong. (Just the name though, everything else is fine).

Oh canon events with Haanja will be drawn that's for sure just debating final scene with the otherworlder, or the new Old men cultists nex. For now just probing for other scenarios.
How do i leave the Cathredal? I have the three gizmos from there and the game suggests returning to Haanja but i cant jump the bridge and backtracking trough the underground just leads to a dead end just before the tentacle area.

Sir you teleport back to Arszenon by opening up the map and clicking the town, talk to Haanja, fix the bridge and then she will appear there.
i didnt realize i can fast travel via the map. That would have saved so much time...
falling in the swamp?
Those commissioned images should be includes in the game. Sounds like a mod

So you have just... walked around the entire game and backtracking... ... without fast traveling... at all?

You can't give birth directly, instead you'll need to be defeated by demi human and eat the suspicious soup, defeated by the slugs when you're about to purge the abomination broodling inside you, defeated by the pig when you're about to run off from the hut, talk to the dark priest when your parasite is at 4 stack, defeated by the cathedral priest 2 times. And how to access it? Just go to the gallery by clicking on the book on the left side of haanja inside the monastery
Talk to Haanja in the convent. You will fight a battle. Win and you'll purge the pregnancy; lose and Lilia will give birth (bad end).
If we're talking about giving birth as a result of the pregnancy status effect, Lilia can give birth in the demi-human den, the pig farm, the Cathedral basement, and the North East Village. These places involve a scene that will result in Lilia getting impregnated, but she can already be pregnant and everything will play out identically.
There are a couple of more birth scenes, but they're parasitic (tentacle cave, swamp boss), and/or strictly tied to a loss scene of a boss (swamp boss, the final boss).

To be honest I was also like that when I first played this game for the first time, I only realized it on the second playthrough in the new game plus
New / Relatively recent games in the past few year? unsure.
The only other games I feel like that are old an comparable is Parasite in City and Anthophobia
