Well I cant seem to trigger the event in the living room at 11:00pm with mom & sis, I've done the part when sis is crying on weekends and I got rid of her bf, sometimes she randomly comes in my room to get fucked and I bang mom basically every morning in the kitchen, I'm not sure what I'm missing at this point but I still get nothing when I enter the living at 11pm?

First time your sister need to knock door of your mother room while you are fucking her at 9 am. . After a while you'll see the scene.

I'm trying the alternate route and I didn't have enough money to bribe the doctor (I offered 10 but didn't have 20). Since then there is no option for me to try to bribe him again. Do I need to edit the save file to get the choice to pop up again, or should I be able to try again? It's been a little over a week in-game and every day there's still no option to bribe again.

Well you and shurik need to spy on her at 21... so you need to finsih spying on his mum during at 21

You shouldn't speak with Shurik until 21:00 (when mom is with her boyfriend). Then go talk to Shurik, and, if you did everything right, an option will appear to go spy on your mom.
