To be fair I don't think StevenCarson's implementation is the best example of this fetish, and he's said he'll be doing less of it in the future because it lost him supporters.

In general the execution usually lets the idea down IMO. The best examples I've seen can more accurately be described as long dick shaped nipples instead of literal dicks that got lost up north and started paying titty rent.
