And to answer Alex's whole question, if his problems were due to psychogenic reasons, he didn't go to the hospital on his own because he simply didn't care and his caregiver Nojiko didn't investigate and didn't pressure him to undergo therapy.
Physical issues are not, nor were they ever, a pre-requisite for erectile dysfunction.
