Looks pretty real to me, fake breasts typically look bolted on, they stick out and don't really sag. The only exception I've seen in Ava Addams who has fake breasts that look natural but they're not anywhere close to the size of Leanna Crow.

I'm really not sure how I feel about all that pregnancy shit. I aint got nothing against preggo lovers or anything of course. I just find the idea of the protagonist fathering so many children and having essentially no interaction with them completely fucked up.

You have the choice to take birth control pills if you're not interested in getting anyone pregnant. Although, I can't recall if it's just on my dev build for now, or if it's part of 0.17.5. Anyways, you can always set player.pregnancy_chance to 0 if that's something you're not interested in. As of 0.17.5, it resets to 20 after a night's sleep, so beware.
Also, yeah there's no real interaction with the child after the first few weeks, but it's not something we can realistically do, like having a child raising simulation, it's just out of the scope of the game. It would also require a bunch of art assets etc, something that we're not going to do due to the sheer amount of work required. Plus, and you know it, people would ask repeatedly to be able to fuck the daughters/sons. You know it, not gonna happen.
If you have any questions about the game and shit, the wiki is pretty detailed and gives full walkthroughs as well as cheats you can use. Check it out. There's a good chance the answer you want is there.

Having just recently finished the recent 0.17.5 update, I gotta say that this new element was very underwhelming and even the update to Diane's story in general was mostly terrible.
I did think that the introduction of possible pregnancy to the sex could be an interesting part of the game given how many women the MC fucks, but I always wondered how the game would deal with it in terms of the other characters reactions and how it would deal with the kids after they're born. This update didn't do either of these things well.
For starters, the reactions of Debbie and Jenny aren't anywhere near what you might expect when finding out that Diane - a single, mature woman - is now pregnant. There's no great surprise or shock, or even any curiosity about the father and how it happened. During the weeks of pregnancy, the MC doesn't have anything they can do with Diane like helping her food cravings, buying baby clothes, or just preparing for the arrival in any way. Each of these things could lead to other sexual encounters too like Maria at the pizzeria (food cravings), Veronica at Consum-R (baby clothes), and even Lucy at the daycare (getting Richard's help to build a crib). I suppose this could all come later, but still, the fact that you do nothing with Diane, not even pregnancy sex, was a bit of a letdown.
As far as when the baby is born, I wouldn't say we need anything major like a SIMS-type system that has the MC raising the child, but the fact that the baby just seems to disappear after a couple of weeks is odd. They can at least acknowledge the baby's existence with conversation dialogue of, "How's the baby doing?", maybe even allow players to name their babies, and even have it in a crib in a room where you can check in on it. Also, I'd say that after the first baby the pregnancy mini-game should be non-existent as the woman will now be on birth control and will tell this to the MC.
Finally, the Diane story update. There were elements that I enjoyed like the topless sunbathing/drinks part being combined into 1 day, the titjob, the threesome, and even the fact that the characters now kiss, but the whole story of her milk production business and barn just felt really ridiculous to me. Granted, this is a work of fiction and there's already a lot of crazy fictional stuff in this game, but tearing down your house to build a barn so you can milk women and then sell the milk is a crazier storyline than Aqua or Daisy. I mean, why not just get the fancy bench/chair thing she has in the barn and put it in the shed? You could even have the Daisy storyline start by having the MC discover the first piece when doing the gardening and then have her be the main source of milk thereby allowing Diane to not have to do it anymore. Since Daisy is a cow-girl, she's more well-suited to producing lots of milk than a human woman. Also, I was really hoping we'd eventually get to have sex with Diane in her own bedroom, but now that her house is gone that will obviously never happen.
Having now seen what this pregnancy part of the game is like, I really hope it doesn't get applied to every single female character. Do it for Jenny since she keeps mentioning it every time you have sex and maybe Roxxy can get pregnant too, but maybe just leave it at that. I know that DC has said before that Debbie would also get a pregnancy update, but maybe put a pin in that one and say that Debbie either had her tubes tied or is now infertile.
