I don't think there's a chance for that, I don't think he'll even try, since the same pattern of relationships between the main characters has been used here for 3 years, reading reviews of his previous works, what is here is most likely his peak, or he really got tired and decided to just go with the flow and finish the game

The small dick one is a valid plot point in my opinion. It is a fetish after all. The other ones not so much. In Japanese games they do go to ridiculous lengths with the stupidity of the main character. At some point the MC has to just accept being a cuck or do something else.

And the second half of the game is a silent war between the spouses, in which the man wants everything to stop, and the woman wants freedom, to feel loved and desired. Depending on the election, this led to her cheating on her husband with some guy while drunk in a bar. But, of course, there are choices that can lead to a good ending. Also in the game, the character has the opportunity to reveal his spouse's infidelity with the help of elections. I can't remember the name, but I played it in 2011 or 2012. Such a game is quite difficult to perceive and the drama is usually very heavy. But the bottom line is that in this game there is not a word about small penises, the characters are not stupid, no one is sick. Unstable relationships between people and a combination of circumstances become the basis for NTR. Now the authors are following simpler paths to justify the NTR.

Based on that brief summary it doesn't sound like a NTR story but a drama story instead. A NTR story needs more than cheating. There doesn't seem be any love between the couple anymore, at least when the cheating happened, and the player choices seems to decide if the man still loves the woman.

Well, that's what I said - it's a dramatic story. The NTR in this game was that in one of the storylines (depending on the choices)At the same time, the woman did not want to divorce her husband and recalled that they had been in love since high school, and at the same time it seemed to her that her husband was too cold and she wanted to feel wanted, and every time she blamed herself and cried after cheating. NTR is not the main theme of this game, although quite a large part of the game is related to it. The way i see it, the problem is when a obscure and simple theme or argument for a story (cheating) turn into a mainstream genre like NTR, all the sudden all original ideas are gone, we have a explosion in works in this genre with almost no creativity, most authors just will follow the safe cookie cutter set of rules for plot line because is easy, is more or less like what happened with Isekai manga/anime all follow the same formula. If we look here in the F95 forum for every 10 games posted 5 or 6 have the NTR tag because is popular but is basically more the same.
But i dont blame only the authors for the lack of creativity it's 50/50 with the public who got used to read/play the same thing over and over.

Fujino, although most of us like his game in some aspect, did the same as many others he just follow the same NTR set of "rules" writing his game with almost no deviation, in the end because his tight 30 days update policy he most of the time used a easy route to meet the deadline.

For instance, Suguha blackmail subplot he made her know the truth, but as soon he wrote this he came up with the " i can't tell this to Kirito because will hurt him" which negate the effect of her knowing the truth that's was a easy route to avoid conflit in the plot. If insted he had did the same without the "hurt him part" the fans would have always that expectation "suguha it's going to tell or going to do something", but because the hurt him part we know she's not going to do anything.
