Best guess I have is the masked lady is the BDSM Maid and Toy is probably an NPC if you like in other words she don't matter. What is, exactly, an NTR route in this particular game? When/how do you enter it? Mild spoilers are fine, though a no spoilers response would be better...
I played TT numerous times, but never got further than 37-67% of the content... my save files always got nuked in one unrelated unfortumate way or another...
