I am not 100% sure what you mean, but It could be the one in the first map by the castle (where you cant do anything atm).
oh, no. It wasn't the castle bit, I don't think? It involved coming face to face with another futa that mentions how you broke the game. I feel like I'm going crazy if it doesn't exist, though. Maybe it had something to do with a broken save, or edited incorrectly?

how is it that I've never seen this one at the castle. What are the requirements for it, or is it Story mode specifically?

This is not from the castle I think and I don't know if its the Error screen one, I can't remember. I unpacked the .jar (you can just unpack it with 7zip) and looked through all the pic's and in my 5 min search that was the only one that stood out.
welp. time to figure out what the QR code is, I guess.

Not surprising, but the effort to make it a QR code is funny.

Is there a way to avoid the mimic? Like, not get captured by it, if the tile ends up being a mimic?

Yes, with sufficiently high PERCEPTION stats, Hiro will not open mimic chests.
