IIRC you need enough Scouting? 4 according to the wiki, though I remember 5... So either boost your perception or take Surveyor. Also check your statuses if some are reducing perception.
Edit: Or just do extra scouting before entering chest tile.

You need scouting level of 4 to avoid them (the little eye icon next to the "shush" icon which is how hidden you are), either click it a few times, put few points into the "Surveyor" perk, get enough perception to increase the level passively or all of the aforementioned.

This shit is so unintuitive and obtuse. It could really use a battle tutorial... particularly for sex. Don't have to explain every possible option but a basic "how to get fucked" and a "how to fuck em" would be outstanding.

You can, volunteer as a sign girl and she lets you as a reward. It's either Story Mode Exclusive or happens on different visits to the shop in Adventure Mode, either way there is indeed a scene of us topping her. There's also the chance to eat her ass, without the need to be the Sign Girl, by Kneeling when prompted. The topping scene requires you to bottom for her customers, then avoid settling as a full-time sign girl and abandoning your quest.

An interactive test/practice fight with a mannequin, that can't do damage but does do attacks, so the player can understand what negates/counters what and how to perform certain stances & actions safely.

There could potentially be a gender change feature that happens in-game (even after initial character creation) if the pronoun choice feature gets developed and implemented, since there is indeed the possibility that Alma's Feminization Training could be tied to it in future. But entire sex changes, no, this is not a game where Hiro will have pusspuss. Alma lost her licence the last time she tried that.
