Finding items quests are not RNGs, they're scripted to appear no matter what at the first try wail the quest is active.
Some will requires you to beat the boss at the end or a forced event mini boss.
try talking to every npcs in town, you probably missed a dialouge telling you to meet someone before going.

From what I've seen on his Ci-en he may add one or two netori(?) characters? But not really since from what I understand she has some kind of open marriage... I'll be honest, I am more than fine with the author not putting any actual NTR in, it is his choice, and I respect it. And if he add the "NTR" only for prostitutes to spite the people who beg him to add NTR in the game, I can only respect him even more.
