You don't. You just tell the game what relationship you want when you name the women you live with at the start. And you can change it at any time in the game by clicking the heart icon in the bottom right.

There is dialogue where mom says "we are mother and son". I imagine this is written into the script when you choose that relationship with her instead of just simple name swapping like many other games.
