You're perhaps running into the issue of man goo being visible when not wearing clothes versus it being hidden when you are: it will depend exactly where on character's body it is. Try using a tissue.
Unless you really mean that it happens as soon as you lose your clothes...in which case, then I'm not sure, maybe you're playing on the buggy developer build like suggested above.
It would be nice if high fitness would reduce the chance of getting bruised in non-forced encounters. It's rather difficult to get banged without getting banged up.

I usually find it while scavving the Junkyard or the Industrial Area though I believe it's a random item.

Probably because somebody provided the changelog for 0.3.3.18, but not the game files for that version.
You're better off not playing the version in the OP either way, it's a dev version and thus extremely buggy. Better to grab the latest stable version (0.3.3.04) from itch.io.
AFAIK the dev will also provide a proper changelog once the next stable version is released.

mentioned a release cycle of 4 - 6 weeks, but that was back in Juli, three months ago. Not sure if any of the non-public posts include an updated release estimate. From what little the post previews show the dev is still working on the exhibitionism storyline.

Oh, joy. A Changelog for ANOTHER buggy developer version. Anything you find via the kemono link posted earlier is not to be shared. A stable build will be pushed when it is deemed to be ready.
Protip: Check Itch.io for the latest stable. Don't download a build that is ANYWHERE else.

After a certain point in the questline, taking too long to complete the next step can fail the quest. I think it's... two weeks or so?
Been a while so i can't say when exactly the clock starts ticking though.

Not sure of the trigger myself either, but I think Mira gets flagged as dead if you don't hit that start trigger in twenty days.
I've always thought that Cass telling you about Mira going missing was the start point.
I keep getting errors when sex scenes are about to happen and always end up with anal. Is there a fix for this?
