It's trans content- for some unknown reason, futa content is much rarer in real porn- and yes, it's completely avoidable. If you decide to go to the pub in the evening, you get an opportunity to hook up with one of 4 women at the pub, and one of them, Natalie, is trans. The choice to hook up with her is highlighted with a reminder (Please note that Natalie is trans) in the game so you can't stumble into it ass-backwards, and it's not necessary for any of the storylines in the game to continue, or tracked behind the scenes to unlock anything.
