Just wanted to drop in to expand a bit on my review. I totally understand the whole "not biting off more than you could chew" for your first endeavor. If the goal is to link the follow ups i'm not sure how you'd explain it in the plot, but I really feel like the story would benefit from expanding the timeline. Giving only 7 days sort of robs a lot of the content you could include. Going from the first blowjob they've ever given to throating it could easily be expanded with days of practice and coaching. Going balls deep for the first anal experience robs you of time you could take first using fingers, then time with plugs in, etc. Its essentially the same with each day of "goals".
Perhaps turning the days into weeks would allow a larger number of interactions for each set goal and gradual progress to be made which would allow the player to get more into the characters.
The last thing I wanted to add is regarding calling the "patient" the F word. Thats a pretty divisive term. You are already offering some distinctive domination/training paths with the different responses you offer. So, perhaps locking that behind the more aggressively dominant/humiliation track or something would make sense. Its entirely possible I'm alone on that island, but I'd really rather not have that included.
Regardless, an impressive outcome for your first effort! I'm looking forward to seeing the sequel(s).
