It's clearly not the dev's first love or main focus (which is fine) but it does receive some new events most updates, incl. this one.
A lot of them revolve around the MC being a prostitute which eh, wouldn't be my preference, but just because it's not particularly to my tastes doesn't mean it's completely abandoned.
The Emily/Adriana choice means players will likely have locked themselves out of some content (with minimal in-game guidance) and refusing to help Leyja may have cut a lot of story events short.

is there a way to up bravery to get the rest of the surgeries or are they just not accessable yet
You can get every surgery except for the facial feminization atm. You don't really need 'bravery' as the game tells you, you need to work at the sex shop / lair until some girl offers you a breast surgery coupon & I think the BNWO storyline for the body contouring iirc

I tried searching but couldn't find actual confirmation, but - specifically on the dom route - are you actually going to be able to have both Emily and Adriana permanently, or do you inevitably have to lose one or the other when Adriana gives you the choice to break up with your respective partners (as an aside, I had that event happen, and the conversation played out as if I had previously said I wanted to pursue that, despite not doing so, during the text exchange where she first brings up the idea, not sure if that's a bug, though)?
And if you will be able to get them both, is it confirmed that you will only be able to do so if you break up with Emily? Do we know if telling Adriana we want to stay casual will fully end her content, or will we continue to have content with her in that case, as the conversation implies (her saying she will "convince us")?
