I have several of those elements in my game, and have made a lot of them optional. At the beginning of each episode I ask about sharing, pissing, futa, and any other questionable fetish that shows up. It's a little more work because I have to rework some of the scenes and dialogue to fit the choices, but since I make a kinetic novel, there's not 100 scenes I have to worry about.
Depending on who is involved, I may have to do up to 3 different sets of animations during sexy-times, but it is what it is.
The only thing people don't get a choice about is lesbian interactions. My game revolves around a female wrestling organization.. ladies are gonna lick labias.
