Same reason why people like eating spicy food: At some point, the pain simply turns into another layer TO enjoy.
We human beings are creatures of sensation. Sometimes, bad feelings can work as vehicles for enjoyable feelings. It is why pain, similarly, is something that people like in sex. Because it makes it makes the sexual release that much more intense.
See, it's not that NTR enjoyers don't feel the mental anguish that would logically follow being cheated upon. It's that that pain, that feeling of having your stomach sink as horror slowly creeps up on you hits harder than a normal orgasm.

I can't say that I will EVER agree with that perspective, but I respect that you've thought this through and can describe it so accurately. There are undoubtedly "knowledge hazards" when it comes to pleasure/sex, and I think there are "points of no return" where you just get so deep into justifying the need for a certain sensation to yourself that it's literally an addiction. I feel like at the very least NTR is a semi-negotiable thing irl (swingers do exist after all), though I wonder if the emotional stress it brings is any better than the genuine health risk turbo-weird shit like scat presents.

That said, so long as no one lives out scat IRL, there are no genuine health risks. I mean, that IS what porn is for, no? Playing out fantasies that you really can't in normal life.
Mind, having difficulties making meaningful real-life connections is the inherent risk of consuming too much of ANY porn, so simply being here is itself an admittance of some risk. Fetishising idealized relationships, regardless of what that ideal is, can be bad for you if you don't separate make-believe from real life.

The corruption and degradation of a character which puts them into debauched situations is hot, and also it's not real. I don't know about others, but I don't translate hentai fetishes to real life, so enjoying incest and NTR doesn't mean I wanna get cucked by my sister wife.

The act of cheating is just a bonus or multiplier, it's the degeneracy and moral corruption of the wife/FMC that's the main course for me. This is why if the female characters in an NTR game don't progressively and noticeably change to be degenerate sluts then it's an NTR game not worth playing.
