It's been a while since I've played it, but I think Simple Days is like that, but with a male MC. The early game is a visual novel with several romanceable characters but the past several updates have been a sandbox taking place after the MC moves in with the successfully romanced character(s). The MC can get the girls pregnant and from what I have read it sounds like they can eventually have 1-2 kids, but I got bored with the sandbox part of the game so I never played to the point where the babies were born.
