There are two answers here imo.

1) Porn will never be as entertaining if it mimicked life 100% realistically. Not many want to see a girl slightly bobbing up and down on a 5 incher. In most situations, realism is just a flavor. Otherwise it would be a simulation. Let's say I want a "realistic" FPS. Ideally that would mean that characters don't take 13 shots to kill. It would not mean that the player needs to start the round by doing in-game cardio exercises and stretches so that they don't pull a muscle running on the map.

But to your point, if there is a market for "realistic" adult games where the sexiness comes at a loss of visual quality or exaggerations, than that is definitely a valid option. Similar to how porn has been starting to cast less DD blow up dolls in favor of girls who actually look like people you run into in your life. Or how amateur style filming is finding more prominence lately.
