In most situations, realism is just a flavor. Otherwise it would be a simulation. Let's say I want a "realistic" FPS. Ideally that would mean that characters don't take 13 shots to kill. It would not mean that the player needs to start the round by doing in-game cardio exercises and stretches so that they don't pull a muscle running on the map.

But to your point, if there is a market for "realistic" adult games where the sexiness comes at a loss of visual quality or exaggerations, than that is definitely a valid option. Or how amateur style filming is finding more prominence lately.
