there was a version that kept your settings as long as you did not close the game, but now you have to set them every run

quick question. in the torture chamber i cant defeat the 1st enemy but i know there are 3 chambers to beat. so how do i defeat the 1st guy. also on top of the mountain there is a clinic asking for a special ticket. how do i get the special ticket?
