i'm already doing that, this is a case of pictures named incorrectly or missing as far as i know, i had already reported this many times bfore, i hoped it would be fixed now

Is this still in active development? See that the OP says it hasn't been updated since 09-05-2021
Currently, mod is effectively on hold due to devs' RL issues. Development process slowly continues, with loserishome regularly rolling out bugfixes, but no big updates are coming in the near future. Despite that, if you haven't tried this mod before, you can start playing without worrying about unfinished content, since mod is nearly complete and any really big updates are reserved for bonus features, like overdue mechanics, breeder ending, Aisha pregnancies and so on.
If I'm not mistaken, front page just wasn't updated visually, but the link does lead to the latest current version, although, methinks, without any bugfixes applied, so, if your game happens to enounter some error, you'll have to dig through latest dozen or so pages to fix them. Fortunately, most of the kinks been ironed out, so they shouldn't plague you any.
Ok lets get this straight maybe I'm wrong..
But need to unlock the requirement first though.. Read the orb I don't quite remember.. I think all cossack(the guy who watches liz give birth) event is finish and preg casino event 2.. Or something
Then follow this step
1. Get her pregnant
2. Sleep until she realise that she is pregnant.
3. Work at front casino
4. The blonde lady will comment about her pregnancy when she play the guessing game
5. Go to arena and fight
6. Sleep until she is in labor condition
7. Go to thugs house?

In Echo's house the requirements are cossak 7 and casino event 2. either way I have all those requirements and more and I've followed those steps exactly, even worked the casino only after she was full-term and then went to the arena and then slept till in labor yet still nada.
Main questline all I'm missing is the clone event and endings
Modded questline I'm missing Birth Lessons, Clone Birth, Inspection 1 (I got 2 and 3, idk why it skipped but whatever), Anxious Sherlaw, White Haired Oliver, and Complainer Matt.
Perhaps there this is a bug?
Anyone got a save compatible with the version listed in the main page?

To add on to this, I you go into the change log you can see that v.5 came out in September 2022.
so is this an alternate game, or do i somehow add this to the original game somehow? if the latter, how do i do that?
anyone got a save? i just finished paying though the regular game and dont want to have to go though it again

Better check the JoiPlay Discord and update your JoiPlay and its plugin.
You get access to Discord by opening JoiPlay > 3 Dot menu in the top right corner > About there is a Discord icon.
