Does every girl have a girlfriend route so far? I've only gotten it with Scarlett, but maybe it was just because hers was the first to trigger. Is it just a matter of getting their relationship high enough?

As far as I remember, I've gotten the girlfriend route with all the main girls.
Do I need a new playthrough for all of them, or is it more in line with other harem games where you can officially date all of the girls?
Everyone except Ashley has a girlfriend option now (Ashley's is planned as an ending). They're impossible to miss, you likely just haven't reached them yet.
Scarlett's is at relationship level 4.
Hannah at level 5.
Both Ezri and BFF are at level 6.
You can date all of them (except Ashley) at the same time!
Thanks but I've figured it out by now. I was actually really close with all of them without even realizing.
I tried this game again... But I couldn't play it again. First of all, the sandbox - I hate sandboxes. Secondly - (if you put up with the sandbox) hints about nothing! I need specific guidance on where and when to go, what to do, etc. There are so few games with a lesbian female protagonist, and it's a pity to lose another one...

Maybe look at the hints on your phone that tell you exactly what to then.

As Buletti mentioned, there is a whole dedicated guide on the phone, tracking your progress and giving you hints on how to progress with every aspect of the game. It is pointed out in the tutorial, but I did revamp the tutorial a little while ago, so you may not have seen it.
As for the sandbox, well, I wanted to make a sandbox ¯\_(ツ)_/¯ if that's not for you then it's not for you. I appreciate you giving the game a go anyway.
