See, I've been asking myself that. The dev wants to do stuff like this, like, you can tell from this, his previous work, and even previous versions of this game, and I was on-board for that, though, a lot of his more vocal patreons and even some people on itch.io and even here aren't happy with it
Then with the most recent rework, it seemed like he was focusing on M/F incest content, or just M/F content in general, then added the sister characters.
And now he's added more MC focused stuff again, it's kinda confusing, though, entertaining to watch. I personally am not subbed anymore because I was kinda fed up with the game being reworked twice, I liked the GZDoom feel! The demon babes in that version of the game were cool! I understand the patreon characters were kinda jarring from a story and setting perspective, but if he really wanted to get rid of them, I would've liked the old gameplay, just replace the girls with some of these new generic vault dwellers people that either have like one scene or just talking sprites that do nothing and are just there to add some life to the game.
