I don't think it matters how many people requested the changes. If people follow your Patreon it is because they like the game YOU are making, that's what they are paying for. There are hundreds of creators around here, I guarantee you will find at least a dozen that are making exactly the game you want to play. So make your game, whatever that is, just make a plan and go for it. Sure, some people will always be bitching and asking for changes (more NTR, no NTR, add harem, add dickgirls, whatever), but if someone decided to spend money on your Patreon is because whatever you were already doing was good enough for them to support you, so stick to your damn guns and follow the vision you had for your game... assuming you had a vision to begin with.
I was curious to see where the game would go because I was interested to learn what that vision was, where were they going with that whole "therapy for troubled girls" concept. The problem here just might be they didn't have a vision to begin with, they were just making filler until the next sex scene and seeing where it would lead. That's an awful way to make a game with a story, but it would explain why someone would be willing to pivot the game so dramatically to address requests that change the core of the game.
