I'm kinda split on the idea of him knowing or not knowing his wife carries her brother's child. Both otions are extremely appealing. I suspect the "him knowing" path would be more interesting game-wise, though. As for the harem idea, I'd say go for it! Some sister would be in it, some would not, but other women would... this would bring more context, more background and make things more interesting. It would also make it harder to keep track of everything though...
