What the couple "represent" depends largely on the path you are on. But to simplify and put it in broad terms, they represent two individuals who are exploring their sexual desires. And depending on the path you've choosen it's either mutual or it's not. Or it's kind of mutual but not quite. It's not supposed to be all black and white, they are swimming in a lot of grey areas and what they are doing is a very extreme from of sexual practice, naturally this will come with some serious life-altering consequences for better or worse. It's up to you I guess to make that judgement. It all depends on what you want to see happen in this fantasy world.
And I'm not sure exactly what you mean by this question - "what reason does the couple have for existing?" It's quite clear that they both enjoy this lust-filled adventure of theirs. Even if you don't buy that they love eachother anymore, fair enough. But Ethan loves getting cucked by Nat and Nat might love cucking Ethan more than she cares to admit. She loves the fact that her husband want to see her get fucked. And she loves the fact that he will eat her out afterwards. Who else is she gonna replace Ethan with? He is still her husband and not some random dude. It won't feel the same with someone else. And Ethan will bow before Nat like no other man would, maybe she is loving that power. No matter how unhealthy and toxic you might consider such a relationship to be, and no matter how unlikely it is to last.
This is not your vanilla relationship anymore, the form of "love" they express for eachother won't be the same either.
the reference to the couple, what reason does it have to exist, is that I no longer see the reason why they should be together, Nat loves to fuck regardless of Ethan, and he demonstrates this in various scenes, even those of sharing, Ethan is there to be the lapdog of I carry over, with the dicks taken nice and big, Nat, Ethan's dick doesn't even feel it, I agree, these are sick, so they go well together because crazy people can only be with their own kind, a world of their own, hahah, who am I to judge the crazy.
