In the short walkthrough it is called "Ethan Kick Nat." You have to do the NTR route that you prefer but have Ethan be at least suspicious if not angry (the Ethan obvious route is not eligible for this) and if Ethan cheats with Jenna this will also be disabled. Once you get to the scene where Ethan discovers Nat having sex with Darrel in the parking lot one of a few different things can happen. Ethan can choose not to confront and will fall into the depressed route. He can kick Darrel's car and have a confrontation where Nat will either choose Ethan or Darrel to go home with. If you choose to have her go home with Ethan, you can have him either forgive her or kick her out. If you forgive her you will go into the Ethan as dominant route. If you kick her out she will go into the regret route. There is also a weird one where she goes with Darrel, comes back later, and bullies Ethan. I think it is called the NTR Nat Dominant route.

I'm thinking the same thing. You'd be better of just making a standalone game with sissification as the main theme at that point. Adding yet another alternate path with a major kink theme would be a lot of work.
I also don't think it would fit very well into the storylines, there has been 0 indication of Ethan being interested in cross-dressing or anything of that nature. You would have to go back to the first few updates and add alternate choices with new renders, having Ethan try out a pair of Nat's panties when she is in the shower or something like this. Tossing it in now would feel very out of place, even if it's "forced" upon Ethan you still need to set it up properly which will require at least a few updates dedicated to just that.

However I can definitely see Ethan in a chastity cage in the near future. Nat will start to dress sluttier and soon she will walk around the house in nothing but panties and a crop top, maybe some new merch from Darrel's gym, what print should they have on them?
