No one said it has to be about incest, but it's disingenuous to pretend that a game which chooses a title that draws attention to this particular aspect of its protagonist isn't doing it to signal certain theme(s) that can be expected. The same way you have dozens of games named with some variations of "Faithful Girlfriend/Wife/Whatever" which, surprise, are about said girlfriend /wife/whatever not being, in fact, faithful. Spoiler: it's never actual surprise.
It's a porn game, of course it's going to be wish fulfillment, just like pretty much all of them. You are not playing these games for their unmatched realism of sexual situations, are you? It has far less to do with the dev not understanding sexual situations, than it does with the dev understanding both the nature of their product and their audience.
I think you're letting nostalgia color your impression of the game, if you believe that Sophia was at any point more than a shallow stereotype of big-titted dumb female MC who gets "corrupted" into wanton behavior by every man, teenager and half of women around her.
(which, don't get me wrong, is pretty sad. But it is what it is)
